Professional fitness testing after rotator cuff repair. Post-op cardiovascular assessment and return-to-activity guidance for shoulder surgery recovery.
Rotator cuff repair requires 4-6+ months of recovery before returning to full activity. During this time, maintaining cardiovascular fitness is crucial-but you must avoid stressing the healing shoulder. VO₂ max testing helps by assessing fitness on shoulder-safe equipment and providing training zones for recovery-period cardio.
Early in recovery, we test on treadmill or bike (equipment that doesn't stress the shoulder). As you progress and receive PT clearance, we can test on more demanding equipment. Testing provides heart rate zones for safe cardio training throughout your recovery timeline.
Months of limited activity can significantly reduce cardiovascular fitness. Testing early in recovery establishes baseline fitness and provides zones for safe cardio that doesn't stress the shoulder.
Retest at key recovery milestones (3 months, 6 months, return to sport) to track fitness improvements and ensure you're ready for increased activity demands.
For overhead athletes (tennis, baseball, swimming), cardiovascular fitness is essential for return to sport. Testing confirms you have the fitness base to support sport-specific training.
Long recoveries are mentally challenging. Maintaining cardio fitness provides a positive outlet and measurable progress when shoulder rehab feels slow.
Treadmill Walking: If your shoulder is stable in a sling and your surgeon approves, walking is excellent low-impact cardio. We can test walking VO₂ max and provide zones for daily walks.
Note: Most patients cannot test this early. Consult your surgeon and PT before scheduling.
Stationary Bike: Once cleared for light shoulder engagement, cycling is ideal. The Wahoo KICKR Bike allows upright positioning that minimizes shoulder stress. We test cycling VO₂ max and provide power/HR zones for safe training.
Treadmill Running: If cleared by your PT and your shoulder is stable, running is an option. Arm swing should be minimal and controlled.
WaterRower: Once cleared for pulling motions (typically 4-6 months), rowing provides full-body cardio that engages the shoulder in a controlled way. We test rowing VO₂ max when you're ready.
Jacob's Ladder XL: Stair climbing with light arm engagement. Good for late-stage recovery when cleared for overhead movement.
Overhead athletes face unique challenges returning to sport after rotator cuff repair:
Tennis demands explosive shoulder movements and sustained cardiovascular effort. Test cycling or running fitness during recovery, then progress to sport-specific training when cleared. Strong cardio base supports the volume of hitting required to regain tennis fitness.
Throwing athletes need excellent cardiovascular fitness for practice and game demands. Maintain fitness on bike or treadmill during recovery. When cleared for throwing, strong cardio base supports the volume needed to rebuild arm strength.
Swimming is often restricted for 4-6+ months post-op. Maintain cardio on bike or treadmill. When cleared for swimming, test in the pool to establish zones for gradual return to training volume.
CrossFit includes many overhead movements. Focus on lower-body cardio (bike, run) during recovery. When cleared for overhead work, testing provides zones for scaling WODs appropriately.
Using your VO₂ max zones (with PT/surgeon clearance):
Important: You must have clearance from your surgeon or physical therapist before testing. We will ask about your surgery date, current restrictions, and PT clearance. Testing is only appropriate when your medical team approves cardiovascular exercise.
This depends on your surgery type and surgeon's protocol. Many patients can test on bike or treadmill at 6-12 weeks post-op. Always get clearance from your medical team first.
We use equipment appropriate for your recovery phase. Early testing uses lower-body equipment (bike, treadmill) that doesn't stress the shoulder. We never test on equipment you're not cleared for.
Yes, if your shoulder condition allows. Pre-surgery testing establishes baseline fitness, making post-surgery comparisons more meaningful.
Test at key milestones: early recovery (if cleared), 3 months, 6 months, and before return to sport. This tracks fitness progress throughout recovery.
Rotator cuff recovery is long, but you don't have to lose cardiovascular fitness. VO₂ max testing on shoulder-safe equipment provides the data you need for smart training during recovery. Schedule your test today (with medical clearance) and stay fit while your shoulder heals.
